Your First Visit
Here’s what you can expect on your first visit to our office:
1. Please arrive 10-15 minutes early to fill out the new patient intake forms, or download and bring the completed forms with you to your appointment.
2. Be sure to bring a valid ID or driver’s license with you, along with your health insurance card if you’ll be using one.
3. If you have any previous medical data (MRI, CT Scan, X-Rays) please bring those documents with you on your first visit or have them emailed to our office ([email protected]) prior to your arrival.
4. Once you’re checked in, you’ll be introduced to your therapist who will then conduct an evaluation to determine what treatments will work best for you on your following visits.
